Naoto Shirogane is transgender because his shadow deals with internalized transphobia and once he voices his self doubt (his shadow calling himself "really a girl") no one in the group respects Naoto's preferred pronouns, immediately switching to she/her, and never give him a chance to truly explain himself, instead opting to push him to "accept that he's a woman"
Naoto Shirogane was afraid of being seen as a child and (because of his dysphoria) as a girl, and him being treated with misdirected misogyny in the past does not mean he was really a girl all along. He went through great lengths to be seen as a boy (more than a cis woman ever would, especially to the point of extremely dangerous chest binding), and if he still wasn't taken seriously it was because he was and is a child (15)
Naoto's romance route is disgusting as to unlock it you must push your views for him to be a woman on him when he is expressing his doubts about his identity.
Things like this do NOT happen on accident. ATLUS knew what they were doing.
Atlus is complicit with the transphobic narrative of Naoto Shirogane. It doesnt matter if they were TRYING to address misogyny in Japanese workplaces what they actually DID was write a trans man who is forced to ignore his true feelings for the comfort of everyone else around him, and imply that trans men are really just struggling with internalized misogyny and that is extremely transphobic.

Not to get into anime gender discourse, but arguing about an anime characterās gender, like Chihiro from Danganronpa or Naoto from Persona, is entirely pointless considering the people who wrote them clearly didnāt, and probably donāt, acknowledge trans people at all, like, in general. Its very clear why the confusion of how to interpret their gender comes up, as the stories resonate very heavily with some trans people, but are ultimately denied representation because the writers just didnāt give enough of a shit to think of transgender people while creating a plotline explicitly about oneās connection with their gender, and instead of actually going somewhere constructive and giving good representation, boomerang right intoĀ āThey actually fuckinā love being their AGAB, and all of that questioning and shit was pointless, maybe at most meant to have a message about sexism or some shit.ā And especially in the case of Naoto, its very clear that heās a trans man, to the point where up until the game makes it very clear heās a cis woman you have to actively try to think heās not trangender (Like, his shadowās whole fucking deal was to physically change his own body so he could be a cis man, its literally right there.) Oh course, the writers then decide to go down a painfully familiarĀ āOh, no, its just because the workforce they wish to be apart of is sexist towards women and no other reason for them to have issues with their gender!ā (See, how people describe people like James Barry) which is just a very regressive view on this matter. So like, it gets stuck at this point where canonically, these characters are cis but are written very much as transgender, and while headcanons are great and all, Naoto will only ever be acknowledged as a cis woman, Chihiro will only ever be considered a cis man, and the writers behind them never have to or want to even think about the implications of these characters and how much it just hurts to not be represented in characters who could be very good pieces of representation if the writers gave half of a ratās shit about transgender people.
In short: Media written about implicatively transgender characters will always suck when written by people who donāt care about transgender people and any discourse about what gender they are is null and void because of this.
Iām sure tons of people have made this exact take but its 4 am and Iām too angry to sleep.
